If you have been assessed as ‘NOT eligible’ for NHS Continuing Healthcare (CHC) funding, following a Decision Support Tool (DST) assessment and are considering an appeal, our DST Analysis service can assist you. A Registered Nurse will review your DST, eligibility decision letter and any panel reports. We will arrange a personalised discussion to understand your views, discuss the evidence used and review the assessment process. We provide clear guidance on the CHC criteria and advise on possible options, including whether to appeal the decision.

Lasting Power of Attorney Certificate Provider

 An independent Registered Nurse will visit you at home to discuss your Lasting Power of Attorney (LPA) in a private and confidential setting. Our service includes a comprehensive mental capacity assessment to confirm your understanding of the LPA, to ensure no pressure or fraud is involved and address any concerns. We provide documentation following Office of the Public Guardian (OPG) guidelines with a thorough review and verification of your LPA. If you have already undergone a CHC Checklist assessment and disagree with the outcome, our CHC Checklist Analysis Service, provided by an experienced Registered Nurse, can offer expert advice and support. We will closely evaluate your completed CHC Checklist to ensure all your health and social care needs are accurately documented. Through personalised consultations, we will discuss your views, review the evidence used, and guide you on the CHC Checklist criteria and available options, such as requesting a reconsideration of the outcome.
