What is NHS Continuing Healthcare (CHC)

NHS Continuing Healthcare (CHC) is the name given when the NHS fully funds a care package for individuals with significant and complex health needs

The NHS covers all costs associated with an individual's care, including medical treatment, personal care, care home fees, equipment, support with daily activities, and any necessary nursing or therapy services

This funding is not means-tested, so the individual does not have to pay for their care.

To get this funding, a person must have an assessment to check if they qualify - For most, the 'checklist' is just the start of the process.

A Registered Nurse will guide you through the NHS Continuing Healthcare criteria and help you prepare for your assessment. We offer personalised support by attending the Decision Support Tool (DST) meeting with you, or representing you if you cannot attend, to ensure a fair evaluation. After the meeting, we provide feedback on the process and advise on potential outcomes and options.

Reviews happen often—during these times, your NHS funding could be at risk of being removed! A Nurse expert in NHS CHC is available to assist you during your review meeting. This service is ideal for people already recieving CHC funding, including Fast-Track, Funded Nursing Care (FNC) and Joint Funding. We offer, in-person and virtual assistance options, including professional guidance and personalised support throughout the review process, along with feedback on outcomes.

Continuing Healthcare (CHC) Checklist & Screening Process 

For those about to undergo the Continuing Healthcare (CHC) screening process, a Nurse will offer expert guidance, helping you to understand the CHC process and funding criteria. We will assist you in preparation for the screening, represent you during the Checklist meeting to ensure your needs are accurately conveyed, we provide a debrief after the meeting to discuss the outcome and next steps.

Decision Support Tool (DST)

Document Analysis

If you have been assessed as ‘NOT eligible’ for NHS CHC funding, following a Decision Support Tool (DST) meeting and are considering an appeal, our DST Analysis service can assist you. Provided by an experienced CHC Nurse, we will closely evaluate your completed DST to ensure all your health and social care needs are accurately documented. Through a personalised consultation, we will discuss your views, review the evidence used, and guide you on the DST criteria and feedback on available options, such as making an appeal - we can help with this too

 Benefit from our professional, confidential and convenient service. Our independent Nurse will discuss your Lasting Power of Attorney (LPA) in a private, we also offer bespoke assessments tailored for individuals with disabilities requiring a Mental Capacity assessment to ensure you fully understand the LPA, confirm that no pressure or fraud is involved and address any concerns you may have. We provide documentation that adheres to the Office of the Public Guardian (OPG) guidelines, including a detailed review and verification of your LPA if need
