What is a Continuing Healthcare (CHC) Checklist
For most people the Checklist is the first stage for accessing Free NHS Continuing Healthcare Funding
It is a screening tool used by the NHS to check if someone should have a full assessment for NHS funding.
It is basically a filter to decide who should have a more detailed assessment called a DST meeting.

How the Checklist Works
Steps in the Process:
The checklist is typically completed by trained health or social care practitioners, such as nurses, doctors, or social workers.
It involves evaluating various health and care needs to see if they meet the criteria for a full assessment.
The pactitioner will score the checklist and make a recomendation to the NHS
The checklist, the documents and the recomendation is then sent to the NHS : This is ussually the ICB
The NHS will then make a decision and write to you with the outcome
Outcomes: There are two possible outcomes
Positive Checklist:
Indicates that the individual should proceed to a full assessment for CHC eligibility.
Negative Checklist:
Suggests that the individual does not require a full assessment and is not eligible for CHC funding at this stage.
