NHS Continuing Healthcare (CHC) is a fully funded package of care provided by the NHS for individuals with significant and complex health needs.  The NHS covers all costs associated with the individual’s care, including medical treatment, personal care, and support with daily activities. This means the individual does not have to pay for their care. To receive this funding, individuals must undergo an assessment to determine their eligibility. For most people a checklist is the start of this process

What is a Continuing Healthcare (CHC) Checklist

The Continuing Healthcare (CHC) Checklist is a screening tool used within the NHS to determine whether an individual should be referred for a full assessment of eligibility for NHS Continuing Healthcare (CHC) funding.

The CHC Checklist is used to ensure that individuals with significant healthcare needs are identified early and accurately. It serves as an initial filter to determine who should undergo a more detailed assessment for CHC funding. This helps streamline the process, ensuring that resources are allocated efficiently and that those in need receive timely support.

The Process

The checklist is typically completed by trained health or social care practitioners, such as nurses, doctors, or social workers. It involves evaluating various health and care needs to see if they meet the criteria for a full assessment.

Outcomes: There are two possible outcomes

Positive Checklist:

Indicates that the individual should proceed to a full assessment for CHC eligibility.

Negative Checklist:

Suggests that the individual does not require a full assessment and is not eligible for CHC funding at this stage.
