Designed specifically for those individuals who are about to undergo the NHS Continuing Healthcare (CHC) assessment process.

Under the supervision of a trained Continuing Health Care (CHC) Nurse Assessor, individuals will be supported to plan, prepare for and practice undertaking a ‘mock’ DST assessment and/or a CHC check list. With the aim of increasing your confidence at any forth coming assessments,enhancing your knowledge and understanding of the processes involved and enabling you to perform to your best ability during the assessment process.

You will connect directly with an experienced CHC nurse assessor via video conferencing, who will provide you with the guidance and support necessary to enable you to navigatethis crucial stage, when you are attempting to access funded health care.

This practical activity will help you understand what to expect during the interview, how to effectively present your case during the actual assessment and how to challenge decisions.

What We Offer

  • Mock Assessments using current Decision Support Tool (DST) paperwork: You will practice undertaking a “mock” DST interview, under the guidance of a experienced CHC nurse assessor. This hands-on experience will help you to understand what to expect and how to effectively present your case during the actual assessment.

  • Evidence: You will understand what “evidence” you and/or the CHC assessor can use to support the assessment process and how to collect this. This evidence you provide can significantly influence the final decision making regarding your eligibility to accessing funding.

  • CHC Checklists:  You will complete a full CHC checklist with professional support and guidance. Our checklists cover all necessary aspects, ensuring you are thoroughly prepared and confident to take the next steps in completing the paperwork with your Social Worker, Doctor or CHC assessor. The checklist will help you to identify your support needs and will include “evidence” to be used during your DST assessment.

  • Expert Guidance: You will receive tailored advice and tips from a seasoned CHC nurse assessor, on best practice for your assessment interview. Our nurse will help you understand the assessment criteria, how to identify your support needs and explain how the assessment is graded to identify your eligibility to CHC funding.

  • Video Conferencing Support:  You will engage in an interactive session via video conferencing, in the comfort of your own home and at a time convenient to you. You will receive real-time feedback and support with advice on best practice during your assessment, common mistakes and questioning decision making.

  • Documents suitable for the real assessment: Upon completion of our session you may wish to choose to be issued with a copy of your assessment presented in a format that can be submitted to the ICB for inclusion in the actual DST as the individuals representative's 'Statement Of Need'. The paperwork also can be used as an aide-memoire to support you during your assessment, helping you to remember important points to include during your meeting with the CHC team.

  • Additionally**: If you plan ahead, you can also submit the completed paperwork as your evidence,prior to your planned DST assessment.
