Plan, Prepare and Practice with a CHC Nurse

A unique & personalised support service for individuals undergoing the NHS Continuing Healthcare (CHC) process and wanting guidance to do it themselves.
Under the supervision of a trained CHC Nurse Assessor, individuals will be supported to plan, prepare for and practice undertaking a ‘mock’ CHC Checklist and/or a DST. With the aim of:-

Enabling you to perform your best to represent yourself & your loved one.
You will connect directly with an experienced nurse via video conferencing, who will provide you with the guidance and support necessary to enable you to navigate this crucial stage, when you are attempting to access funded health care.
This practical activity will help you understand what to expect during the interview, how to effectively present your case during the actual assessment and how to challenge decisions.
What We Offer

Video Conferencing Support: You will engage in an interactive session via video conferencing, in the comfort of your own home and at a time convenient to you. You will receive real-time feedback and support with advice on best practice during your assessment, common mistakes and questioning decision making.

CHC Checklists: You will complete a full CHC checklist with professional support and guidance. Our checklists cover all necessary aspects, ensuring you are thoroughly prepared and confident to take the next steps in completing the paperwork with your health or social care professional. The checklist will help you to identify your support needs and will include “evidence” to be used during your DST assessment.

DST 'Mock' interview using current Decision Support Tool paperwork: You will practice undertaking a “mock” DST interview, under the guidance of a experienced CHC nurse assessor. This hands-on experience will help you to understand what to expect and how to effectively present your case during the actual assessment.

Expert Guidance: You will receive tailored advice and tips on best practice for your Checklist or DST interview. Our nurse will help you understand the criteria, how to identify your support needs and explain how the forms are graded to identify your eligibility to CHC funding.

Evidence: You will understand what “evidence” is collected to support the assessment process and how to collect this. The evidence you provide can significantly influence the final decision making regarding your eligibility to accessing funding.
Boost Your Evidence:
On completion of the session you may wish to choose to be issued with a copy the DSTdiscussion presented in a format that can be submitted as evidence for inclusion in the real DST as your 'Statement Of Need'.
Additionally**: The 'Statement of Need' document can be used as an aide-memoire to support you throughout your DST, helping you to remember important points that can often be missed during long difficult meetings.
